Application form
The Application Form is quite straightforward - just fill in the appropriate sections for whichever award you are claiming.
Name
If this is a first time application it is most important to write your name exactly how you want it to appear on the certificate that you will receive.
This will then be printed on the certificate in the form [NAME] , [callsign]
eg. A.N.Other, G9QXZ.
You may see some of the certificates and stickers by looking at the 'Certificate Information' page. (Accessible from the 'Award' main page.)
Award Title
For your first award in any category this needs to be unambiguous - once you have made a claim and are in the 'Awards Records' then it can be traced.
Endorsement
You may claim endorsements for 'mode', specific locations, contacts made during your first year of operations etc.
It is generally advisable to consult the Awards Manager regarding the suitability of such endorsements.
Additionally ALL QSL cards must support this claim!
Address Panel
This is used as the address label for your certificate, or if you have forgotten to include an envelope for return of other material.
